Devon County Council has developed NHS Health Check posters for use in GP practices. A poster is available, inviting patients to attend when invited. Please click on the link to view and download the poster:

NHS Healthcheck invitation letter
Public Health Devon has used behavioural insights from the national team to put together a localised invitation which has been approved by the Local Medical Committee. Please upload and amend the necessary fields for your practice and send these letters with an information leaflet (see resource below).

NHS Health Check Information Leaflets
Public Health Devon have created a local information leaflet that should accompany the invitation letter. These leaflets can be ordered through Public Health Devon. Alternatively GP practices can chose the national NHS Health Check information leaflet which is available free of charge from DH publications order line. Translated versions of the leaflet will be available to download and there is an easy read version this should be sent with the invitation letter.
